Control Micro Systems, Inc. (CMS) will pay Laser Job Shop owners a one-time 3% commission for any sales lead (referral) which results in a purchase order for a CMS laser system. Only the first purchase order from a referral, for a single, specific laser application, is commissionable. Subsequent sales to a referral for any reason or purpose are not commissionable and the original purchase order must be received by CMS within 1 year of the date this form is submitted. Referrals to accounts currently maintained by CMS are ineligible. Commissions will be paid when CMS receives payment from the referral. Please complete and submit the form below for consideration. CMS will email or mail confirmation that your submission is eligible for the sales lead compensation program. Thank you for your participation!
